CAREMINDr and Wellness West Partner to Bring Remote Wellness Monitoring to Vulnerable Patients Across Chicago’s West Side

CHICAGO, December 5, 2023 – CAREMINDr, a leading developer of remote patient care technology and Wellness West, a coalition of Chicago’s health and safety-net institutions, announced today a partnership to bring daily monitoring of disease states to the most vulnerable populations on the West Side of Chicago.

The partnership will allow qualifying participants in the Wellness West disease management program to stay in daily or weekly contact and data sharing with their care team through a mobile app on their phone. The CAREMINDr mobile-enabled solution allows patients to monitor and record key health data such as blood pressure and glucose levels and have it sent in real time to care teams. The technology is being rolled out to Wellness West patients who are managing diabetes, hypertension, or depression.

This frequency of information and data sharing would be impossible if left to the face-to-face appointments. Wellness West has collected over 16,000 health risk assessments from community members across the West Side. Twenty-five percent of respondents report having transportation challenges in their daily life, which creates risks for missing medical appointments.

A Proven Innovation for Better Health Outcomes
New research about the efficacy of CAREMINDr remote monitoring was recently presented at American Heart Association’s Hypertension Scientific Sessions. The study found that more than half of patients with uncontrolled high blood pressure brought it under control after participating in remote monitoring.
